A movie about a hockey player who gets hit by a hockey puck and dies?

I saw this movie a very long time ago, so I know that it is not recent. But, in the movie a hockey player (maybe the goalie) gets hit in the head by a hockey puck. He then dies from this injury and goes to heaven, where it is determined (for some reason) that he will return to earth. He does not return as himself, but is instead put in the body of a woman. Once back at earth he finds his girlfriend/fiancee/wife person and they become friends. That is all that I remember, but I was wondering if anyone knew the title.
